As a Bioanalyst High-Complexity Laboratory Director, I've worked with dozens of clinical labs seeking to maintain QC compliance. Many of the forms seen here are used to help manage QC processes in those labs. 

I've taken the 'road-less-traveled' by developing forms as Java-Scripted PDF's, and perhaps the only person doing this for Lab Calculations.  The templates are best used by supervisors and testing personnel on computer desktops within the lab. No program other than Adobe Acrobat Reader, or the newer Acrobat DC Reader is needed.
